Responsibilities:
The Ambulatory Care Pharmacist provides pharmaceutical care services using approved protocols in collaboration with other health professionals. This includes managing drug therapy, providing patient education, and preparing and/or overseeing the preparation and dispensing of medications. The Ambulatory Care Pharmacist promotes high quality, evidence based, cost-effective pharmaceutical care. He/she performs these duties in the clinical areas assigned.
Responsibilities include, but are not limited to the following:

  • Works as part of an interdisciplinary team consisting of other healthcare providers (i.e. physicians, nurses, etc) and healthcare staff.
  • Upholds Synergy Pharmacy Solution’s policies and procedures, principles of responsibilities, and applicable state, federal and local laws.
  • Conducts patient medication history inquiries upon referral from physicians and other medical personnel.
  • Synthesizes a treatment plan based on a given diagnosis, which may include drug therapy and lifestyle modifications, and transitions within the continuum of care settings.
  • Identifies, resolves, and prevents potential and actual medication-related problems including: untreated indications, improper drug selection, sub-therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication.
  • Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record.
  • Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing.
  • Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADRs, eliminating unnecessary drug use and duplication.
  • Provides patient education related to the individual's disease state, including predictable actions, side effects, reasons for proper compliance, and any special instruction.
  • Ensures ongoing performance improvement of the pharmaceutical care services provided.
  • Provides education to providers and staff via presentations, newsletters, formal utilization reviews, and one-on-one interactions.
  • Performs other duties as required.

Qualifications:

  • Doctor of Pharmacy Degree (Pharm.D.), issued by an accredited school of pharmacy
  • ASHP-accredited pharmacy residency, or 3 years experience, or board certification (preferred).
  • Currently licensed and in good standing with the California State Board of Pharmacy.
  • Comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, disease state management, age appropriate therapy, and other information as pertains to medical or pharmaceutical care management plan.
  • Expertise in drug utilization review.
  • Knowledge of all applicable Federal/State laws/regulations regarding delivery of pharmacy services and scope of practice of a pharmacist.
  • Excellent verbal and written communication skills
  • Ability to multitask and work under pressure.

DUTIES:

a. Perform the Full Spectrum of Services as a dispensing pharmacist. Pharmacist shall have knowledge of the full range of professional principles and practices of pharmacy.  

b. Apply approved disease management protocols to external prescriptions. Work therapeutic interchange according to law and policy.  

c. Communicate protocols/therapeutic change requests with physician's offices. Take therapeutic changes over the phone as required in accordance with law and standards of practice.  

d. Have skill in counseling and teaching patients and significant family members regarding drugs. The pharmacist must have knowledge of authorized pharmaceuticals, their desired effects, side effects, and complications of their use. 

e. Have knowledge of the drugs and their possible reactions in order to clearly advise the patient accordingly of possible adverse drug side effects requiring medical intervention. 

f. Serve as drug information source for patients and hospital staff, providing timely responses to such requests. Counsels patients, as needed, on the appropriate use of their prescribed medications.  

g. Ensure Pharmacy Service work areas are secured when departing workplace.  

h. Participate in pharmacy staff in-service meetings when scheduled within normal duty hours.  

i. Perform pharmacy professional position-related administrative duties. 

j. Verify the content and correctness of all recorded entrees and transcribed reports. 

k. Provide and document medical advice to patients by telephone as appropriate.

l. Ensure all equipment is properly cleaned and stored at the end of each work shift. Ensure preventive maintenance and routine cleaning are performed as directed.

m. Ensure a safe work environment and employee safe work habits.

n. Use systems including CHCS, AHLTA, etc., as required by the 75 MDG. Must comply with corresponding 75 MDG training requirements.

o. Must be capable of verifying no less than 60 prescriptions per hour in a high ops tempo environment. 

p. Be productive and perform with minimal supervisory direction.

 

Pharmacist performing services at the MTF shall comply with Government/MTF regulations, Memorandums of Understanding, Operational Plans, etc. regarding administrative services, record keeping, patient referrals, interactions with Government personnel, MTF security, Quality Management/Utilization Management participation, Government property, etc. All regulations, policies, and standard operating procedures will be made available to all personnel for familiarization and to ensure compliance in performance of services. Additionally, personnel shall attend all Government provided orientation and training necessary to perform daily duties (e.g. Composite Health Care Systems (CHCS) training, suicide prevention, customer service, career field training, etc.) on a Government installation.  

Dress and Appearance:  Personnel will maintain a neat well-groomed professional demeanor and appearance including hair, makeup, clothing, and actions. No shorts, sleeveless shirts, or exposed midriffs. Clothing will include comfortable business or casual attire, which will not interfere with duties or infection control and safety guidelines and standards. No facial piercing, other than ear, is allowed, including tongue piercing. Nails will be in accordance with Air Force (AF) and MTF policies. Any visible tattoos must not be offensive. Hospital badges will be issued to all personnel and must be worn while working inside the MTF during duty hours and removed when outside the MTF.

Personnel shall wear clothing in a neatly groomed manner suitable for a professional milieu. Clothing shall be clearly distinguishable from all U.S. military uniforms.

Personnel shall wear an identification badge, on the right upper front of outer clothing at all times while performing duties. The badge shall include the person’s full name, degree, title, and company name.

Brooks Balaris started out 20 years ago in an independent pharmacy in Paso Robles and trained as a pharmacy technician. After over a decade of experience with Longs they were acquired by CVS. By this time, any trace of an independent pharmacy in town was gone having been consumed by corporate giants.

And so was born Brooklin Oaks. Now only a short (and quick!) year later the homegrown and only independent pharmacy in town has grown faster than anyone could have suspected or even dreamed. Brooklin Oaks is not only taking new customers each and every day, but is actually on the cusp of being self sustaining! (3 years ahead of the expected time line!) The Department of Family and Preventive Medicine at The University of Utah School of Medicine offers a Ruth L. Kirchstein National Research Service Award (NRSA) Primary Care Research Fellowship designed to train professionals for careers in research and academic medicine. Two slots are available for 2016–2017, and the fellowship may start in either August 2016 or January 2017.

Eligibility: Qualified individuals must be U.S. citizens, non-citizen nationals, or lawful permanent residents of the United States with completed doctoral training (PhD, MD, DO, DDS, DMD, DNP, PsyD, PharmD or comparable doctoral degree) from an accredited domestic or foreign institution. Masters-trained nurse practitioners and physician assistants are eligible with at least 2 years of clinical experience

Program Details: During this two-year training program, trainees will complete the requirements for the Master of Science in Public Health (MSPH) degree. Trainees will also rotate with faculty research teams, conduct their own research within an established research team, and attend leadership training.

  • Thorough knowledge of OTC and prescription medications.
  • Proficiency with Microsoft Word, Excel, and PowerPoint.

The suitable applicant must have a current Utah Pharmacy Licenseand be in good standing with the Division of Occupational and Professional Licensing. The successful applicant will be a qualified pharmacist with a background in community pharmacy, exceptional product knowledge. Specialized Summer Internship Program in Health Outcomes
The 2016 AMCP Foundation/Allergan, Inc.

The 2016 AMCP Foundation/Allergan, Inc. Specialized Summer Internship Program in Health Outcomes is now accepting applications for two student pharmacists. The program’s goal is to create awareness of career paths in health outcomes and pharmacoeconomics research, which are the foundation for evidence-based decision-making in managed health care systems. In addition, the interns will gain knowledge of the pharmaceutical industry, managed health care systems and professional association management.

Internship Details

Two selected student pharmacists will spend ten weeks at the Allergan Global Health Outcomes Department in Irvine, California, where they will complete a health outcomes-related project under the supervision of a pharmacist preceptor. The results will be exhibited at AMCP Nexus 2016. In addition, interns may spend one of those 10 weeks in a managed care organization to gain a basic understanding of formulary development, implementation, maintenance, evaluation and the importance of health outcomes in the formulary decision-making process.
The program will cover housing, travel and other approved expenses. Interns will also receive a modest stipend. The internship must be completed within a 10-consecutive-week period between May and August 2016.

Internship Requirements

Interns must be enrolled as a full-time Doctor of Pharmacy degree student in an ACPE-accredited school or college of pharmacy during the 2015-2016 school year, with anticipated graduation in 2017 or 2018. They must be eligible for employment in the United States.
Besides completing an online application, applicants must submit two letters of recommendation and a Letter of Good Academic Standing from their institution.

Application deadline

Application deadline is Monday, February 15, 2016. The online application must be completed; both letters of recommendation and the Letter of Good Academic Standing must be postmarked on or before Monday, February 15.

San Luis Valley Health is seeking a full time Pharmacy Director to oversee all aspects of the Pharmacy Department including:
