Becoming a Training Site

Utah Sites

If your site has not yet been used for training students from the College of Pharmacy, the following process will be completed to confirm your site.
  • The Experiential Education Director and preceptor will have an introductory phone call to discuss the rotation in more detail and address any questions.
  • A Clinical Training Agreement must be completed and signed by both parties before moving forward (this can take several months). The University of Utah General Counsel must approve the agreement before it is signed.
  • Once the Clinical Training Agreement is signed, the preceptor(s) will need to complete and submit all preceptor requirements in CORE ELMS.
  • The Experiential Education Office will complete an introductory site visit in Utah before the first rotation or within the first year. For out-of-state sites, the EE Office will inquire about the site’s most recent quality assurance visit from the college they precept for.

Out-of-State Sites

If your out-of-state site has not yet been used for training students from the College of Pharmacy, the following process will be completed to confirm your site.

The process is the same as above for out-of-state sites, with two exceptions.  The Preceptor must already be an active Preceptor for another school of pharmacy.  Rather than conducting a site visit, the EE Team will inquire about the site’s most recent quality assurance visit from the college they precept for.
